13 4.1 Topical corticosteroids with antimicrobials

• As doctors, we know only too well how dangerous and addictive these drugs are and the serious side effects of steroids.

Trafficking offences carry maximum sentences of 14 years’ imprisonment and a fine. It is illegal to possess them without a prescription or to supply or produce them without a licence. Health professionals who are not specialists in MS, including some GPs and A&E staff, may not realise that a high dose of steroids is needed to treat a relapse.

Talk to your doctor before taking other medicines

Cyclophosphamide has the potential to cause
serious short term side effects including bladder irritation (cystitis)
and bone marrow suppression (low white cell count) leading to infection. Mesna (a uroprotectant) is often given to patients receiving injections
of cyclophosphamide to reduce the risk of bladder problems. Patients
may also be advised to drink a lot of water to flush out the bladder but
this should be discussed with the medical team. Other short term
effects include nausea and vomiting and hair loss. People mentioned a number of short-term side effects, including nausea/vomiting, headaches, mood swings and flatulence. Longer term effects known to occur include weight gain and puffiness, particularly of the face, which one woman described.

Additional important advice

If you take more than your prescribed dose of steroid tablets, contact 111 for advice. The side effects will usually pass once you finish the treatment, but do not stop taking your medicine without speaking to your doctor. Your doctor will advise you on how to gradually reduce your dose if necessary.

If testosterone deficiency occurs during fetal development, then male characteristics may not completely develop. If testosterone deficiency occurs during puberty, a boy’s growth may slow and no growth spurt will be seen. The child may have reduced development of pubic hair, growth of the penis and testes, and deepening of the voice. How do I take steroids?

Live vaccines are made using weakened, live bacteria or viruses. In general, you shouldn’t have live vaccines while you are on steroid medication and for 6 months afterwards. This is likely to be longer (at least 24 months) if you have had a stem cell transplant. Avoid contact with people who have chicken pox or shingles, even if you have had these infections in the past – they could be serious if you have lymphoma and are on steroids. Tell your doctor straightaway if you think you could have chicken pox or shingles.
